About this role

To lead the Active Care Service, the full-time Active Care APP Manager will oversee clinical and operational aspects of a team of Advance Care Providers, ensuring high-quality primary care delivery while maintaining a limited panel of clinical visits in a remote environment. Key responsibilities Provide ongoing coaching and support to APPs, ensuring strong clinical reasoning and member-centered care Monitor performance and quality oversight, identifying trends and implementing improvement plans Collaborate with primary care leadership to design and implement effective workflows for the Active Care Service Required qualifications Prior primary care clinical background with strong management experience 3+ years NP clinical experience in relevant care settings Demonstrated experience leading change and supporting clinician development Active, unrestricted RN + APRN licensure and national certification Fluid usage of the Google suite of products
