About this role
To ensure quality care for patients, the full-time Oregon and Washington Licensed Care Coordinator will manage patient assessments and care interventions, coordinate care transitions, and collaborate with clinical teams while working remotely. Key responsibilities: Coordinate patient care by collaborating with clinicians, reviewing treatment plans, and making referral recommendations Ensure compliance with policies and regulations while documenting treatment plans and facilitating care transitions Manage patient data and records, conducting quality management studies and reporting on care metrics Required qualifications: Minimum two (2) years of experience in behavioral health case management or care coordination Master's degree in Psychology, Counseling, Social Work, or a related field with three (3) years of relevant experience Licenses required from Oregon and Washington within six months of hire National Provider Identifier required at hire
