About this role
To support population health initiatives, the full-time Population Health Assistant will coordinate care processes, manage data and resources, and assist in training while working remotely Monday through Friday from 8 AM to 4:30 PM. Key responsibilities Coordinates intake and facilitates the Care Coordination process by verifying health plan eligibility and triaging cases Researches and maintains resources, performs medical literature reviews, and assists in developing reports for analysis Facilitates the implementation of new employer groups and care gap outreach, ensuring accuracy and compliance with established protocols Required qualifications High School Graduate Typically requires 5 years of experience in population health management or related fields Proficiency in ICD-10, CPT, and HCPCS coding preferred Advanced skills in Microsoft Office (Excel, PowerPoint, Word) and data management Knowledge of medical terminology and ability to maintain confidentiality
